The Midwifery Team at Canopy has been taking care of pregnant people & their babies on the North Shore for the last two decades. Midwives are primary care providers for pregnancy, labour & the early postpartum period. We are FULLY FUNDED through MSP, so there is no cost to you. We have extensive experience in postpartum care. If you are interested in one-on-one care for you & your baby we are happy to help.
What Postpartum Midwifery Care Looks Like:
A midwife will see you & your baby at your home from birth up to 6 weeks postpartum. These visits include well checks for you & baby
For YOU: blood pressure assessments / vaginal delivery & CS recovery checks / hands-on feeding support / emotional well being assessments.
FOR BABY: regular weight checks / feeding plans / exams
Streamlined referrals to specialists as needed for you or your baby.
At 6 weeks your care will be transferred back to your GP or if you do not have a GP we can refer to you a Primary Care Provider.
How to sign up:
Since you are being seen by another provider for your Pregnancy & Labour care you can SELF-REFER to our group for your Postpartum Care at anytime. Submit THIS FORM any time during your pregnancy or just after you deliver ++Please fill this out on a COMPUTER, not your phone++.
Next Steps:
Once we have received your form please expect a call from us within a few days to confirm receipt. At this time we will collect some info from you & give you next steps to get your Postpartum Plan in place & make your visits as seamless as possible after your birth.
